Windang Family Medical Centre ("the practice", "we", "us") is committed to protecting the privacy of our patients and website visitors. We handle personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and the Health Records and Information Privacy Act 2002 (NSW).
This policy explains how we handle personal information generally, and specifically how information is collected and used through this website (windangfmc.com).
Your health information
As a general practice, we collect personal and health information from our patients so that we can provide safe, high-quality medical care. This includes information such as your name, contact details, Medicare details, medical history, medications, test results and correspondence with other health providers. This information is collected directly from you wherever possible, stored securely in our clinical software, and accessed only by members of our team who need it to deliver or support your care.
We only share your health information with third parties — such as specialists, pathology and imaging providers, My Health Record, or Medicare — with your consent, or where required or permitted by law.

Information collected through this website
Contact form. If you use the enquiry form on our website, we collect the details you provide — your name, email address, phone number and message — and use them solely to respond to your enquiry. The form is intended for general enquiries only. Please do not include medical or health information in the form. For clinical matters, appointments, results or prescriptions, please call the practice.
Our contact form is operated by Formspree Inc., a third-party form processing service based in the United States. Information you submit through the form is transmitted to and processed on Formspree's servers overseas before being delivered to us. Formspree's handling of that information is governed by its own privacy policy.
Online bookings. Appointment bookings made through the "Book Online" links on this website are handled by HotDoc Pty Ltd, an Australian online booking provider. Information you provide when booking is handled in accordance with HotDoc's own privacy policy.
Website hosting and technical data. This website is hosted on GitHub Pages (GitHub Inc., USA). Like most web hosts, GitHub may log technical information about visits, such as IP addresses, for security and operational purposes. Our website also loads fonts from Google Fonts and displays an embedded Google Map; when these load, your browser sends standard technical requests (such as your IP address) to Google.
Cookies and analytics. This website does not use advertising cookies and does not run analytics or tracking scripts.
How we hold and protect information
Enquiries submitted through the website are delivered to our practice email, which is managed on a secure, professionally administered platform. Patient health records are held in our clinical information system with access controls, and our team is trained in privacy and confidentiality obligations. We take reasonable steps to protect all personal information we hold from misuse, interference, loss, and unauthorised access, modification or disclosure.
Access and correction
You have the right to request access to the personal information we hold about you, and to ask us to correct it if it is inaccurate, out of date or incomplete. Requests can be made to our reception team or Practice Manager. We may need to verify your identity before providing access, and in some circumstances a reasonable fee may apply for providing copies of records. We will respond to requests within a reasonable time.
